The measurable trade-offs are a small weight penalty over an equivalent step-over frame and slightly more flex when the bike is heavily loaded with rear panniers or a child seat.

The reliable way to judge capacity is the manufacturer's stated weight limit rather than assumptions about frame shape. Step-through models in this bracket publish limits comparable to their step-over equivalents, and several are rated for 120 kg or more of total load.

Can a step-through e-bike carry a rear child seat or heavy panniers?

Yes, provided two separate ratings are respected: the rear rack's own capacity, typically around 25 kg on commuter models, and the bike's total weight limit, which covers rider, cargo, and accessories combined. A child seat mounts to the rack or frame, so the rack rating is usually the binding constraint.
